The Release of Information (ROI) process enables healthcare providers—and clients themselves—to access health records in accordance with the Health Records Act 2001 and the Health Services Act 1988.
Requests by General Practitioners and Specialists
General Practitioners and Specialist Clinicians requesting medical records to support the care of mutual patients must submit a written request on their official practice letterhead. The request must include:
- Patient details: Full name and date of birth
- Information requested: Clearly specify the documents or details needed
- Signed patient consent
Requests by Clients
Patients have the right to request access to their own health records. To do so, the following must be submitted:
- A completed SHQ Authority to Release/Obtain Health Information Form
- A certified copy of photo identification (must be dated within the last 12 months)
- Details to help identify the information requested, such as:
- Date(s) of attendance
- Service area attended
- Type of document requested (e.g., test results, summary)
If you have changed your name or use an alias, please include all known names to assist with locating your records.
Charges for Information Release
SHQ may charge a fee for:
- Releasing informationprocessing the request.
- Producing a copy of the client’s health record
- Preparing a summary report
- Charges will be communicated prior to processing the request.
